your facility. VPM provides treatment for individuals dealing
with medical, emotional and psychiatric issues in their current
home. The program operates as an emotional and behavioral
assessment and life adjustment service as well as an extension
of post psychiatric hospitalization or partial hospitalization.

Adjustment Disorders
VPM conducts initial assessments on adults moving to community
living environments to assist with transitional issues that may
arise as part of the adjustment process of moving. We provide
support groups at facilities and can perform family therapy as
well.
Licensed Clinical Social Workers conduct individual, couple,
family and group psychotherapy, initial assessments, provide
psychiatric diagnosis and treatment plans to individuals
residing in independent living, assisted living communities,
nursing homes and other group home settings. Psychiatrists and
Nurse Practitioners provide medication evaluation and management
on site.
